TalentBurst is recruiting a Registered Nurse Case Manager in Philadelphia, PA to provide clinically focused care coordination and transition management during hospitalization. You will collaborate with patients, families, physicians, social workers, payers, and the care team to facilitate timely discharge planning and safe care transitions.

The role requires at least five years of inpatient case management experience, strong discharge planning, and the ability to work independently on onsite

Formación

• RN required.
• Minimum of 5 years of relevant experience.
• Prior inpatient hospital case management experience required.
• Strong discharge planning experience required.
• Hospital-based case management experience required; insurance-only case management experience will not be considered.
• Bachelor's degree required.
• Strong technical proficiency and comfort navigating healthcare systems and technology platforms.
• Ability to work independently with minimal onboarding or supervision.
• Experience managing patient caseloads of approximately 18-25 patients daily.
• Excellent communication, coordination, and interdisciplinary collaboration skills.

Responsabilidades

• Care Coordination & Transition Management: Assess patients and families for transition of care needs; develop, implement, and evaluate individualized discharge plans; initiate discharge planning on admission and manage progression.
• Post-Acute Care Coordination: Coordinate home care services, DME, infusion, rehabilitation, SNF placement, and other resources as appropriate.
• High-Risk Patient Management: Identify high-risk patients and develop targeted interventions.
• Responsibilities Cont'd: Monitor progress toward discharge milestones and escalate barriers as needed.
• Other Responsibilities: Adhere to departmental policies; pursue ongoing professional development; maintain safety, infection control, and professional standards.
