Qualifications

• Bachelor of Arts or Science is required.
• Registered Nurse: bachelor’s degree preferred; Master Level Social Worker: MSW from CSWE accredited school; Social Worker: Bachelor of Social Work or related field.
• Experience: a minimum of three or more years of acute care clinical nursing.

Responsibilities

• Coordinate discharge planning with the multi-disciplinary team to ensure seamless transitions.
• Review insurance benefits and discuss co-pays / deductibles and identify barriers to care.
• Assess patient needs and resources within 24–48 hours of admission and develop discharge plans with input from patient/family.
