• Collaborate with payers to optimize transition to appropriate level of care.
• Document thoroughly in the medical record and maintain HIPAA compliance.
• Advocate for patients and families to support informed decisions and best clinical outcomes.
• Assist in discharge planning, care coordination, and post-discharge resources.
