Job summary

Penn Medicine seeks a Registered Nurse to support discharge planning and utilization management, coordinating home care, DME, hospice, and community resources with the care team.

You will educate patients and families, document in the Canopy system, and ensure timely discharge while maintaining patient dignity and privacy. A PA RN license and Bachelor’s degree in Nursing are required, with 2+ years of clinical experience preferred.

Qualifications

• Registered Nurse – PA license required.
• Bachelor of Arts or Science (Required).
• Bachelor's Degree in Nursing required; 2+ years clinical experience preferred.

Responsibilities

• Monitor utilization management activities and coordinate discharge planning with the care team.
• Coordinate home care, DME, placements, hospice and transportation.
• Educate patients and families about scope and limitations of benefits.
• Document all activities in Canopy and communicate discharge plans.
• Participate in daily interdisciplinary meetings and ensure timely patient flow.
