Job summary

Terros Health is seeking a Discharge Planner to join our interdisciplinary team at the Mitchell Recovery Health Center in Phoenix. This role coordinates discharge planning for individuals with serious mental illness or general mental health needs, interfacing with inpatient and outpatient teams to ensure a smooth transition.

Responsibilities include reviewing clinical information, coordinating services, and supporting follow-up with the clinical team post-discharge.

Qualifications

• GED/High school diploma with a minimum of 4 years of behavioral health employment experience.
• An associates degree in the field of behavioral health and 2 years of behavioral health employment experience.
• A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience.
• A bachelor's degree in the field of behavioral health or a master's degree or higher in the field of behavioral health.

Responsibilities

• Review presenting clinical information and identify resources to coordinate discharge.
• Coordinate inpatient and outpatient services and service planning.
• Connect inpatients with ongoing outpatient treatment after discharge.
• Work with hospital staff, outpatient team, family, PCPs, and health plans to ensure effective transitions.
• Participate in individualized service planning and execution of discharge plans.
