Job summary

Terros Health seeks a Case Manager to join an interdisciplinary clinical team at the South Mountain Recovery Health Center in Phoenix. This full-time role includes a comprehensive benefits package and opportunities for professional growth.

You will conduct assessments, develop service plans, coordinate care with families and providers, and pursue best-practice outcomes for clients with serious mental illness. Training is provided for new graduates with a behavioral health degree.
