Job summary

Terros Health is seeking a Discharge Planner to join our interdisciplinary clinical team servicing individuals with Serious Mental Illness or general mental health needs at our Mitchell clinic in Phoenix. This full-time role coordinates inpatient and outpatient discharge planning, connects clients with ongoing outpatient services, and supports family, PCPs, and health plans as needed.

Responsibilities include reviewing clinical information, planning service delivery, and reducing readmission

Qualifications

• GED/HS diploma with a minimum of 4 years of behavioral health employment experience.
• An associates degree in the field of behavioral health and 2 years of behavioral health employment experience.
• A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience.
• A bachelor's degree in the field of behavioral health, or a master's degree or higher in the field of behavioral health.

Responsibilities

• Review presenting clinical information, identify resources, and help coordinate discharge into the community and follow up with the clinical team.
• Activities will be performed in inpatient and outpatient settings (inpatient visits, coordination meetings, service planning).
• Connect individuals receiving Level I inpatient services with ongoing outpatient treatment after discharge.
• Participate in individualized service planning and execution of written discharge plans.
• Offer coordination to an array of services that address recovery goals.
• Reduce the risk of readmission and improve coordination with PCP and Terros psychiatric providers.
