Job summary

Terros Health is seeking a Case Manager for the Mitchell Recovery Health Center in Phoenix to support individuals with serious mental illness or general mental health diagnoses under court-ordered treatment. Role requires coordinating with clinical teams to implement service plans and engage clients in goal attainment.

Position is full-time, Mon-Fri 8:00am-5:00pm, located at 40 E. Mitchell Dr., with full benefits, PTO, and 401(k). New grads welcome; bilingual pay differential available.

Qualifications

• High school diploma or GED with 1.5 years behavioral health employment exp.
• Associate or higher degree in medical/behavioral health fields.
• Bachelor's degree with BH experience or relevant BH degree.
• Master's or higher in behavioral health preferred.

Responsibilities

• Assist clients to identify treatment goals and provide supportive services.
• Complete assessments, crisis plans and service plans with the team.
• Coordinate care with member, family, and providers as planned.
• Conduct outreach to ensure resources are available and engaged.
• Document and track case management activities and outcomes.
