The Terros Health Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site.
The TERROS GMH/SA Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site Manager.
