Job summary

Rotech Healthcare is seeking a Hospital Liaison to connect hospital staff with home care services. You will coordinate discharge planning for respiratory and DME needs, arrange equipment delivery, and educate patients and families on proper use.

The role includes collaboration with case management, nurses, and physicians, with travel to patient homes as needed and potential bonus opportunities.

Qualifications

• Experience coordinating patient discharges and DME delivery.
• Knowledge of medical terminology and billing practices preferred.
• Valid driver’s license with clean driving record when applicable.

Responsibilities

• Coordinate discharge planning and equipment delivery at hospital discharge.
• Educate patients and caregivers on equipment use and care.
• Maintain accurate documentation including referrals, tickets and logs.
• Support emergency equipment needs and troubleshoot issues.
