Responsibilities

• Coordinates with RN Case Manager and Social Worker for support and communicates updates.
• Provides clerical support of multidisciplinary rounds and updates Tempo.
• Makes referrals through the web-based referral system to post-acute care providers.
• Schedules outpatient hemodialysis appointments.
• Monitors PAC referrals and expedites placement.
• Prioritizes workload for timely PAC contact and intervention.
• Sends information to PAC providers as requested.
• Communicates PAC status with RN Case Managers/Social Workers and patients.
