Las Palmas Medical Center in El Paso, TX, is hiring a Registered Nurse Case Manager for weekends only. This role coordinates discharge planning and patient care across the care team, ensuring continuity and high-quality outcomes for a defined patient population.

The position requires TX RN licensure (or Compact) and an Associate Degree or BSN. Prior acute care case management experience is preferred, with strong collaboration and education duties to support informed decisions and

Qualifications

• RN licensure in TX or Compact State
• Associates Degree or BSN
• 3–5 years clinical experience in acute health care or case management preferred
• Knowledge of case management philosophy, utilization review, and discharge planning

Responsabilités

• Guide patients and families through program orientation, rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and recovery goals
• Complete psychosocial assessments and discharge planning evaluations, document findings, and communicate needs across the care team
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promote HCA Healthcare values of compassion, respect, and excellence in patient interactions
