Job summary

HCA HealthONE is seeking a Case Management Assistant to support discharge planning and post-acute placement coordination. The CMA will work with the RN case manager or social worker to facilitate patient discharges and document progress throughout the day.

The role involves clerical duties, referrals through PAC systems, and maintaining communications about PAC status and bed availability. Requires an associate degree and healthcare experience.

Qualifications

• Associate degree preferred.
• Medical knowledge and terminology required.
• Minimum of one year experience in a health care setting required.

Responsibilities

• Coordinates with RN case manager and social worker for support needed.
• Provides clerical support in multidisciplinary rounds, including updating NATE Tempo at direction of team.
• Makes referrals through the web-based referral system to post-acute care providers requested.
• Monitors patients with PAC referrals and intervenes to expedite discharge.
• Prioritizes workload to ensure timely contact and intervention with PAC providers.
• Delivers PAC referral information and corresponding patient communications.
