Job summary

La Palma Intercommunity Hospital is seeking a Case Management/Discharge Planning professional to coordinate activities of the Case Management Department under the supervision of the Case Manager/Social Worker. You will assist with discharge planning, referrals, and Medicaid submissions, ensuring safe and efficient patient discharges.

Required is knowledge of discharge planning terminology, basic to intermediate computer skills, and a high school diploma; LVN or associate degree preferred.

Qualifications

• Knowledge of discharge planning and case management terminology and functions in managed care and non-managed care environments.
• Experience in basic to intermediate computer skills.
• High School Diploma required.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans and referrals.
• Maintain Medicaid state submission forms for presentation to on-site reviewer.
• Assist with Discharge Planning as assigned by interviewing patients/families and gathering data for safe discharge plans.
