Job summary

La Palma Intercommunity Hospital in La Palma, CA seeks a Case Management professional to coordinate discharge planning under the guidance of the Case Manager. You will assist with referrals, authorizations, and Medicaid form submission while coordinating data to support safe discharge.

The role emphasizes collaboration with hospital teams, data collection, and development of patient-focused discharge plans. Prior CM terminology knowledge and basic computer skills are needed.

Qualifications

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions.
• Experience in basic to intermediate computer skills/knowledge.
• High School Diploma required.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker and assist with discharge plan development and implementation.
• Assist with case management referrals and authorizations and maintain Medicaid state submission forms for review.
• Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe, patient-focused discharge plan.
