Job summary

Las Palmas Medical Center in El Paso, TX, seeks a Registered Nurse Case Manager PRN to coordinate patient care and discharge planning. You will review medical records for utilization, liaise with patients, families, and hospital staff, and ensure smooth handoffs to post-acute care networks.

Ideal candidates hold TX RN licensure, an Associate or BSN, and 3–5 years acute care case management experience. Demonstrated knowledge of utilization review, InterQual criteria, and strong communication are

Qualifications

• RN license in TX or Compact State required
• Associates Degree or BSN required
• Three-five years clinical experience in acute care case management preferred
• Knowledge of case management philosophy/process and discharge planning preferred
• InterQual or similar criteria familiarity and strong data interpretation skills desired

Responsibilities

• Coordinate delivery of medical services and discharge planning for a defined patient population.
• Perform concurrent and retrospective reviews for utilization review and payer requirements.
• Serve as liaison with patient/family, facility staff, physicians and community agencies.
• Ensure continuity in handoffs of patient information between hospital and other care entities.
