AdventHealth in Zephyrhills, FL is seeking a dedicated RN Care Manager to coordinate discharge planning and post-acute transitions. You will join a multidisciplinary team to review patient status, educate families, and ensure timely, safe care following hospitalization.

The role requires an active RN license in Florida, with 2+ years of hospital nursing experience. Bachelor’s degree preferred; CCM/ACM credentials are a plus. Strong leadership and data analysis skills are essential.

Qualifications

• RN licensed in Florida with required certifications.
• 2+ medical/hospital nursing experience.
• Bachelor’s degree preferred; CCM/ACM preferred optional.

Responsibilities

• Actively participates in rounds to review patient status and discharge plans.
• Educates patients and families on emotional, social, and financial impacts of illness.
• Assesses readmitted patients for reasons and rebuilds discharge plans.
• Facilitates family care conferences and coordinates post-acute care with payors.
• Documents plans, barriers, and progression in the medical record and care coordination.
