Job summary

Qualifications

• High School Graduate / GED.
• Minimum of two years' experience in a health care setting.
• Medical knowledge and terminology.

Responsibilities

• Coordinate with RN Case Manager and Social Worker for discharge planning and post-acute placement.
• Assist with referrals and payor authorizations; communicate status to care team.
• Maintain PAC referral timelines and document processes in Case Management systems.
• Arrange outpatient services (e.g., hemodialysis) and coordinate transfers.
• Provide clerical support for multidisciplinary rounds and patient communication.
