Qualifications

Responsibilities

• Collaborates with physicians, nurses and the care team to plan care from admission to discharge.
• Leads discharge planning and coordinates patient transfers to other facilities.
• Helps patients and families navigate post-acute options and insurance plans.
• Organizes care conferences with patients, families and care team.
• Intervenes in crisis situations and coordinates Baker Act if needed.
• Coordinates with government agencies (DCF, APS, CMS) for safe post-acute care.
