Job summary

HCA Florida Lake City Hospital in Florida is seeking a qualified Social Worker to promote patient-centered care by evaluating psychosocial needs and coordinating discharge planning across ICU, PCU, Med/Surg, and ER Units. You will collaborate with the care team, identify barriers to discharge, assist patients and families in navigating post-acute options, and educate on community resources and coverage.

Master's degree required; inpatient experience preferred.

Responsibilities

• Collaborate with R.N. case managers, nurses, physicians and insurers to coordinate discharge.
• Identify and address barriers to discharge across financial, emotional, psychosocial, medical and spiritual domains.
• Perform assessments and develop discharge plans for high-risk patients.
• Coordinate meetings with the care team, patients and families to discuss care plans.
• Educate patients and families on community resources and post-acute care options.
