Job summary

HCA Florida Memorial Hospital in Jacksonville, FL is seeking an experienced Registered Nurse Case Manager to coordinate care across the patient journey and optimize discharge planning. You will apply clinical expertise and communication skills to promote safe, efficient care transitions.

The role requires Florida RN licensure with 1–3 years of hospital nursing, and 2 years in case management are preferred. BSN is highly regarded and certification in case management is a plus.

Qualifications

• RN licensure in Florida or multi-state license
• 1–3 years hospital nursing experience required
• 2 years in Case Management preferred
• Case Management or Utilization Review certification preferred

Responsibilities

• Coordinate plan of care for patient stay and discharge planning
• Facilitate progression of care and transitions between settings
• Conduct psychosocial, medical, and discharge assessments
• Ensure appropriate post-acute medical needs and resources with Social Services
• Escalate progression of care issues using approved criteria (InterQual)
• Refer patients to third-party payers and disease/case management programs
• Act as liaison between physicians, patient/family, staff, and outside agencies
