Responsibilities

• Interview residents/patients and complete assessments within 72 working hours.
• Obtain insurance recertifications and document in billing software.
• Attend interdisciplinary team conferences and family conferences.
• Discharge planning to community services including home health and skilled nursing facilities.
• Provide Medicare/insurance notices and maintain client progress notes weekly.
