Mountains Community Hospital is actively seeking a compassionate Case Management Assistant to support discharge planning, care coordination, and patient transitions. You will perform administrative duties, organize referrals to post-acute providers, and help maintain accurate electronic health records under the Case Management team’s guidance.

Qualifications

• High school diploma or equivalent.
• Healthcare admin experience preferred.
• Experience in hospital, medical office, case management or discharge planning preferred.

Responsibilities

• Provide administrative and clerical support to the Case Management Department.
• Assist with discharge planning activities and coordination of patient services.
• Coordinate referrals to post-acute providers and community resources.
• Organize information for patient transitions and discharge arrangements.
• Communicate with patients, families, providers, and insurers as appropriate.
• Maintain accurate documentation in electronic health records and files.
• Track referrals, authorizations, discharge arrangements, and follow-ups.
• Assist with patient placement information, transportation, and community services.
• Gather information from outside facilities to support care coordination.
• Protect patient information and comply with HIPAA and privacy rules.
• Respond to phone calls and relay messages to Case Management staff.
• Assist with reports, data entry, and other admin tasks.
