Job summary

HCA Healthcare, Inc. is seeking a Case Management Assistant in Colorado to support discharge planning and post-acute placement coordination. The CMA assists the RN case manager and social worker, tracks PAC referrals, and handles clerical tasks to ensure smooth patient transitions.

The role emphasizes timely communication with PAC providers, updating on progress, and ensuring patient questions are routed appropriately. Relevant healthcare experience and knowledge of case management are essential.

Qualifications

• Associate degree preferred.
• Medical knowledge and terminology required.
• Minimum of one year experience in a health care setting required.

Responsibilities

• Assist RN case manager or social worker with patient discharges requiring post-acute placement or services.
• Identify and report barriers to discharges and expedite patient flow.
• Provide clerical support in case management and multidisciplinary rounds.
• Make referrals through the web-based system to PAC providers and prepare required packets.
• Monitor PAC referrals and bed availability; communicate progress to the care team.
• Deliver Medicare messages and essential discharge documentation to patients.
• Coordinate transportation and transfer center details for PAC transfers.
• Maintain knowledge of Case Management processes and regulatory requirements.
