The role balances patient needs with resource utilization and begins discharge planning at admission, reassessing throughout hospitalization. The Coordinator serves as the primary contact for discharge planners and social workers, leads problem-solving for complex cases, and collaborates with leadership on

Qualifications

• RN or social work licensure in Colorado or multistate license.
• Graduate of an accredited nursing or social work program.
• 5+ years clinical experience; UM/CM experience 3-5 years; 1-3 years lead/supervisory experience.

Responsibilities

• Primary contact for discharge planners and social workers; coordinate plan of care and post-discharge needs.
• Provide expertise for problem-solving complex cases, process improvements, and monitor length of stay/barriers to discharge.
• Collaborate with Director on staff evaluation, coaching, counseling, and development of action plans.
• Develop and supervise new staff, including orientation, core competency evaluations, and performance counseling.
