Job summary

HCA Florida Capital Hospital in Tallahassee is seeking a Case Management Assistant (PRN) to support discharge planning and post-acute services alongside the RN case manager and social worker. You will help identify barriers, coordinate care, and manage administrative tasks within Case Management.

Qualifications

• CNA/MA experience required.
• High School Graduate / GED.

Responsibilities

• Assist with facilitating patient discharges requiring post-acute placement or services.
• Coordinate with RN case manager/social worker and report discharge barriers.
• Provide clerical support and maintain Case Management records and systems.
• Make referrals to PAC providers and update patient status and bed availability.
• Monitor Medicare messages and deliver required notices per policy.
