UofL Health is seeking a Case Management Assistant to support the Care Coordination Department. The CMA collaborates with RNs, social workers, patients, families and the healthcare team to coordinate care, communicate, and facilitate timely referrals and discharge planning.

Qualifications include a high school diploma, 1 year healthcare experience, BLS-CPR, and preferred Kentucky Nurse Aide certification. The role emphasizes patient advocacy and accurate documentation in a fast-paced hospital

Competenze

• High school diploma or equivalent required.
• Minimum of one year experience in a healthcare setting; hospital setting preferred.
• BLS-CPR certification required; Kentucky Nurse Aide Certification preferred.
• Knowledge of care coordination, discharge planning, and patient advocacy.

Mansioni

• Collaborate with RN Case Manager, Social Worker, patients, families and healthcare team to coordinate care.
• Deliver MOON letter and explain to Medicare patients; document notice provided.
• Coordinate discharge needs and post-acute referrals with internal and external providers.
• Maintain open lines of communication with case managers and social workers; document referrals.
• Attend department meetings; protect patient information and adhere to HIPAA requirements.
• Complete all routine referral documentation and assist with physician signatures as needed.
