Job summary

Endeavors seeks a Disaster Case Manager to support individuals and families affected by disasters in their recovery journey. You will conduct outreach, assess needs, develop recovery plans, and connect clients to available resources and services.

The role requires documentation accuracy, adherence to program requirements, and collaboration with multidisciplinary teams. Travel, nights, weekends, and holidays may be required to meet business needs.

Qualifications

• Bachelor’s degree in behavioral sciences, human services, or social services preferred.
• High School diploma or GED with three years of relevant experience required (in lieu of degree).
• Experience in disaster-related, child welfare, or recovery environments preferred.

Responsibilities

• Conduct outreach to disaster-affected clients.
• Complete eligibility assessments and the intake process.
• Conduct home and community visits to provide ongoing support.
• Collaborate with clients to develop recovery plans addressing unmet needs.
• Monitor progress and assess service effectiveness with follow-ups.
• Assist clients in securing available benefits and community resources.
• Liaise with family services, health professionals, and stakeholders.
• Maintain accurate case histories and progress documentation.
• Document case information using online software and prepare reports.
• Regularly meet with the Case Management Supervisor to review caseload.
