Job summary

Gardant-Management-Solution seeks an experienced nursing supervisor to lead a care team in delivering high-quality resident services. The role focuses on assessments, care planning, and coordinated clinical operations to ensure compassionate, compliant care.

You will mentor staff, foster communication with physicians and families, and drive quality and safety initiatives in a supportive community setting.

Qualifications

• Must hold a current state nursing license (LPN).
• Two years of nursing supervisory experience required.
• Strong written and verbal communication skills.
• Ability to mentor, motivate and inspire team members.

Responsibilities

• Conducts initial assessments of prospective residents and develops individualized care plans.
• Performs wellness assessments and delivers health promotion and self-care education.
• Leads the clinical team, hires and trains staff, and schedules coverage.
• Coordinates infection control and medication management programs.
• Ensures compliance with privacy and confidentiality regulations.
• Supports quality assurance and performance improvement initiatives.

Responsibilities:Conducts initial assessment of prospective residents, including off-site visitsPerforms wellness assessments and develops individualized care plans based on the needs and preferences of each residentDelivers health promotion services, offers disease prevention counseling, and provides self-care education to residentsEngages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experienceFosters communication by regularly engaging with physicians, pertinent healthcare providers, and family membersHires, trains, schedules, and leads the clinical team in providing exceptional care experiencesCoordinates the Community’s infection control program while maintaining state regulatory complianceDevelop and oversee the Community’s medication management programContributes toward quality assurance and performance improvement initiativesComplies with all rules and regulations regarding confidentiality and privacy of resident informationQualifications:Maintains current state nursing licensure.
