Responsibilities

• Manage and coordinate the admissions process from referral through admission.
• Build and maintain relationships with hospitals, case managers, social workers, and other referral sources.
• Communicate with prospective residents and families throughout the admissions process.
• Coordinate with clinical and administrative teams to facilitate incoming admissions.
• Assist with census development and identify opportunities for growth.
• Ensure timely follow-up on referrals and maintain admissions documentation.
