Job summary

Prairie Community Services in Roseau, MN is seeking a Direct Support Provider to supervise, care for, and support individuals in daily living activities, recreation, community orientation, medication management, meal preparation, laundry, and cleaning. Starting wage is $16.75-$18.75 per hour.

The role is part-time and on-site at Crestwood Home, Roseau, MN, with benefits including health coverage and paid time off. You will work closely with individuals to promote independence and safety.

Qualifications

• Minimum age 17 years or as licensure requires
• Must pass DHS background clearance
• Must have valid driver’s license
• Must be able to read, write, communicate and understand English
• Read, comprehend, and follow each person served’s program related plans and procedures
• Deliver support to the person served on principles of person-centered services
• Administer CPR/First Aid when needed
• Chart daily program information

Responsibilities

• Provide supervision, care and safety to individuals served
• Assist with daily living activities, recreation, community orientation
• Assist with medication management, meal preparation, laundry and cleaning
• Document daily program information and progress
• Follow individual plans and procedures faithfully
• Support individuals with dignity and respect
