Job summary

Prairie Community Services is seeking a Direct Support Provider to supervise and assist individuals in our residential and community-based programs in Minnesota. The role requires compassionate care, basic daily living support, meal prep, and safety oversight on a part-time, on-site schedule.

The starting wage is $17.25–$18.25 per hour, with CPR/First Aid training and a focus on person-centered service. DHS background clearance and a valid driver’s license are required to begin.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Support daily living activities, recreation, community orientation and meals.
• Manage medications as directed and chart daily information.
