Job summary

Prairie Community Services is seeking a dedicated Direct Support Provider to supervise and assist individuals in daily living activities, recreation, and community orientation at our on-site location in Minnesota.

This part-time, on-site role offers starting wages of $17.00 to $18.00 per hour and includes benefits such as paid time off, health coverage, and retirement options. Join a mission-driven nonprofit that values respect and person-centered care.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served in daily living, recreation, and community orientation.
• Assist with medication management, meal preparation, laundry and cleaning as needed.
• Support individuals in accordance with their plans and procedures, using person-centered approaches.
• Document daily program information to ensure accurate records.
