Job summary

Prairie Community Services in Minnesota is seeking a Direct Support Provider to supervise, care for, and ensure the safety of individuals in our residential and community programs. You will assist with daily living tasks, recreation, community orientation, medication management, meals, laundry and cleaning.

Starting wage is $17.00-$19.00 per hour. We offer paid time off, comprehensive health coverage, an HSA contribution, life insurance, retirement options, and other voluntary benefits.

Qualifications

• Minimum age 17 or as required by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, communicate and understand English while conducting business.
• Read and follow each person served’s program plans and procedures.
• Deliver support based on person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living tasks, recreation and community orientation.
• Support medication management, meal preparation, laundry and cleaning.
• Follow person-centered plans and procedures.
• Document daily notes and program information.
• Maintain safety and well-being of residents.
