Job summary

Prairie Community Services in Roseau, MN is seeking a Direct Support Provider to supervise, care for and ensure safety for individuals served. Responsibilities include daily living activities, recreation, community orientation, medication management, meal preparation, and charting daily progress. Starting wage is $16.75-$18.75 per hour.

This is a part-time, on-site role. Qualifications include being at least 17, passing DHS background clearance, valid driver’s license, and English literacy.

Qualifications

• Minimum age 17 or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand English.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served across daily living, recreation, community orientation, medication management, meals, laundry and cleaning.
