Job summary

Prairie Community Services is seeking a Direct Support Provider to supervise and assist individuals in daily living activities, recreation, and community orientation at our on-site locations in Minnesota. Responsibilities include medication management, meal preparation, laundry, and maintaining a safe environment; starting wage is $17.00-$18.00 per hour.

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals being served that include daily living activities, recreation, community orientation, medication management, meal preparation, laundry, and cleaning.
• Follow person-centered service plans and document daily progress.
• Administer CPR/First Aid when needed.
• Maintain on-site safety and well-being of individuals served.
