Job summary

Prairie Community Services in Minnesota is seeking a Direct Support Provider to supervise and assist individuals in daily living, recreation, and community orientation. You will support medication management, meal prep, laundry, and cleaning, with a starting wage of $17.00–$18.00 per hour at a part-time, on-site schedule.

Responsibilities include following person-centered plans, reading and communicating in English, and documenting daily progress.

Qualifications

• Minimum age 17 or as required by licensure.
• Pass DHS background clearance.
• Valid driver's license.
• Read, write, communicate in English.
• Follow each person served’s program plans and procedures.
• Deliver support per person-centered plans.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities, recreation, and community orientation.
• Medication management, meal preparation, laundry and cleaning.
• Document daily program information.
