Job summary

Prairie Community Services is seeking a Direct Support Provider in Minnesota to supervise, care for, and assist individuals in daily living activities, recreation, and community orientation. Starting wage is $11.41 per hour, with strong emphasis on person-centered supports.

Qualifications include being at least 17, DHS background clearance, a valid driver’s license, and English proficiency to read and follow program plans.

Qualifications

• Minimum age 17 years or as licensure dictates.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand English.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served in daily living, recreation, community orientation, and meals.
• Assist with medication management, transportation and daily living tasks.
• Document and chart daily program information and progress.
