Job summary

Prairie Community Services is seeking a Direct Support Provider to supervise, assist with daily living activities, and ensure safety for individuals served. The role includes meals, recreation, medication management, and community orientation, with a starting wage of $17.00–$18.00 per hour.

This is a part-time, on-site position in Minnesota. The candidate should meet age, licensing, and background requirements and be able to communicate in English.

Qualifications

• Minimum age 17 or as required by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, communicate and understand English in business.
• Read, comprehend, and follow each person served’s program plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served across daily living, recreation, community orientation, medication management, meals, laundry and cleaning.
• Deliver support using person-centered principles to individuals served.
• Chart daily program information.
