Job summary

Prairie Community Services in Minnesota is seeking a Direct Support Provider to supervise, care and ensure safety for individuals served in daily living activities, recreation, community orientation, meal prep, and housekeeping. Starting wage is $17.00-$19.00 per hour and the role is full-time and on-site.

You must be at least 17 or as required by licensure, pass DHS background clearance, hold a valid driver’s license, and communicate in English.

Qualifications

• Age 17+ or as licensure dictates.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand English.
• Read, comprehend and follow each person served’s program plans and procedures.
• Deliver support to the person served on person-centered principles.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Support daily living activities, recreation, community orientation.
• Assist with medication management, meal prep, laundry and cleaning.
• Document daily program information and progress.
