Qualifications

• High school diploma or equivalent preferred
• Six months of experience in human services preferred
• Must be 18 years of age
• Working knowledge of computers

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Direct Support: provides direct support or assistance per service plans; socialization and behavioral development, personal care, housekeeping, recreation, transportation, shopping, citizenship, and daily living.
• Records: accurately documents progress and activity; reviews records and logs; maintains confidentiality.
• Management of Individuals’ Assets: makes cost-effective purchases within the budget and preferences of the individual.
• Relationships: maintains healthy relationships with individuals, families, guardians and case managers; follows Sevita standards.
• Reporting: reports abuse/neglect and incidents per policy; follows external requirements.
• Rights: maintains confidentiality and respects rights; follows universal precautions.
• Health Care: accompanies to medical appointments; relays orders; administers or supports self-administration of medications; documents delivery.
• Meal Preparation: prepares meals according to planned menus and individual choices; monitors mealtime and special diets.
• Employment Responsibilities: training; scheduling; teamwork; workplace safety.
• Maintenance: operates vehicles safely; housekeeping; minor repairs; safety checks.
• Other: performs other duties; MPA for timekeeping.

Education

High school diploma or equivalent
