Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. This mission-driven team supports residents and community-living individuals, typically serving 4–6 individuals in a program, or up to 10 in periodic settings, through direct care and coordinated activities.

The role is non-exempt, hourly, and can be full-time or part-time.

Qualifications

• Direct support per individual service plans and daily living activities.
• Accurate documentation of progress and privacy maintained.
• Manage purchases within the individual's budget and preferences.
• Maintain professional relationships with individuals, families, and case managers.

Responsibilities

• Deliver support per individual service plans, including health maintenance and skills development.
• Assist with daily activities: personal care, housekeeping, transportation, community integration, shopping.
• Document progress and review records while following confidentiality.
• Report any incidents and protect rights according to policy.
