Job summary

REM Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. This role covers direct support within residential programs, assisting with daily activities, health care, and community integration.

The position involves documentation, medication administration support, and adherence to safety and confidentiality standards; driving and transportation may be required, with state trainings completed as mandated.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.
• Valid driver’s license in good standing; driving may be required.
• Car registration and insurance if providing transportation.

Responsibilities

• Deliver support per individual service/program plans including socialization and health maintenance.
• Assist with daily living activities: personal care, housekeeping, transportation, community integration, shopping.
• Document progress and stay up-to-date with changes in service plans; maintain confidentiality.
• Administer or support administration of medications as directed; report errors.
• Maintain confidentiality and rights of individuals; follow safety and recording procedures.
• Follow safety policies; participate in trainings; ensure a safe workplace.

Skills

Working knowledge of computers
