Job summary

Prairie Community Services in Morris, MN seeks a Direct Support Provider to supervise and assist individuals in daily living, recreation, and community orientation. The position requires compassion, reliability, and adherence to person-centered plans.

Starting wage $16.50-$17.50 per hour with a strong benefits package including health coverage, HSA contributions, life insurance, retirement options, EAP and flexible scheduling.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living tasks such as meal prep, laundry, and cleaning.
• Support recreation, community orientation, and transportation as needed.
• Medication management assistance.
• Observe and document daily program information.
• Follow person-centered plans and procedures.
• Administer CPR/First Aid when needed.
