Job summary

Oak Hill in Farmington, CT is seeking a Direct Support Professional to support individuals with intellectual and developmental disabilities in a residential setting. The role offers a 40-hour workweek, Tuesday through Saturday, 3:00 PM to 11:00 PM, with a base pay of $20.39 per hour and differential pay for specific schedules.

Responsibilities include assisting with daily living activities, personal care, health monitoring, medication administration, and community transportation.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional stays beyond scheduled shift to ensure coverage.

Responsibilities

• Support individuals in living healthy, meaningful, and fulfilling lives by assisting with daily living activities.
• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor health-related information and vital signs when required; document appropriately.
• Administer medications and maintain medication records per training and directives.
• Follow individualized health care plans and support overall well-being.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements; assist during meals.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and medication administration in the EHR.
• Follow Oak Hill, DDS, DPH, HIPAA, and regulatory requirements.
• Respond to emergencies and behavioral needs; apply first aid/CPR when required.
• Collaborate with team; participate in meetings and ongoing professional development.
