Job summary

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene per individual plans.
• Monitor health information and document as required; administer medications per training and directives.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and assist with meals per menus and dietary needs.
• Document daily activities and health information in the EHR and follow regulatory requirements.
