Job summary

AHRC Nassau in Plainview, NY is seeking dedicated Direct Support Professionals to empower individuals living in Community Living Services. You will support daily activities, promote independence, and ensure health, safety, and wellbeing through personalized assistance.

You will work as part of an interdisciplinary team, document plans, mentor new staff, provide transportation, and help individuals engage in meaningful routines and community activities.

Qualifications

• High School diploma or equivalent required.
• Valid NYS driver’s license maintained throughout employment.
• Ability to understand written and oral directives.
• Physical aspects may include lifting and transferring individuals.

Responsibilities

• Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life.
• Assist with self-care routines, health, safety, and household chores while promoting independence.
• Support community integration and maintain natural supports and relationships.
• Assist with medication administration per AMAP policy and procedures.
• Transport individuals to medical appointments, shopping, and recreational outings.
• Attend interdisciplinary team meetings and document progress.
• Mentor new Direct Support Staff and participate in house meetings.
• Perform other duties as assigned by supervisor.
