Job summary

AHRC Nassau is seeking a dedicated Direct Support Professional to provide ongoing support to individuals in the Community Living Services program, helping with self-care routines, health, safety, and daily activities to promote independence and well-being.

You will work as part of an interdisciplinary team, assist with meals and transportation, and document plans and progress while adhering to agency policies and staffing requirements.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license.
• Must have the ability to communicate and understand written and oral directives/information.

Responsibilities

• Provide ongoing guidance and support to individuals to promote independence and engagement in daily routines.
• Help individuals participate in the community and maintain relationships.
• Assist with household chores and meals as needed.
• Follow treatment plans and safety equipment to support health and safety.
• Transport individuals to medical appointments and activities.
• Document progress and maintain records for plans and outcomes.
