Job summary

AHRC NASSAU in New York seeks a Direct Support Professional to provide ongoing guidance and support to individuals in the Community Living Services program. You will assist with self-care, health, safety, household chores, socialization and overall wellbeing.

The role requires carrying out treatment plans, accompanying individuals to appointments, and collaborating with an interdisciplinary team; a NYS driver's license is required and a high school diploma preferred.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in every aspect of their lives.
• Provide support to become active members of the community and maintain/build natural supports and relationships.
• Ensures a safe and clean home by assisting with household chores and by participating in said chores as needed.
• Follows all treatment plans and utilizes safety equipment needed for each individual.
• Supports individuals in preparing meals by teaching concepts such as diet adherence, portion control and cooking skills.
• Assists individuals with medication administration according to policy and procedures.
• Implements and documents all treatment plans and related materials to assist individuals in achieving independence.
• Adheres to overtime, scheduling, and minimum staffing policy to ensure quality support.
• Serves on the inter-disciplinary team by attending meetings to discuss progress and goals.
• Provides transportation to medical appointments, shopping, and recreational outings.
