Responsibilities

Provides ongoing guidance and support to individuals to ensure active participation in daily life and independence.
Supports individuals to become active community members and maintain natural supports.
Ensures a safe and clean home by assisting with household chores and participating as needed.
Follows treatment plans and uses adaptive and safety equipment to promote health and safety.
Supports individuals in meal preparation and cooking skills, including portion control.
Assists with medication administration according to policy and procedures.
Documents treatment plans, medical notes, and progress to support valued outcomes.
Attends meetings as part of the inter-disciplinary team and shares information for progress.
